Imaging studies: Ultrasound or MRI scans can provide detailed images of the anal area and the fistula. Treatment: Surgery is usually necessary.Anal fistulas rarely heal on their own, so surgery is the most common and effective treatment. The main goal of surgery is to eliminate the fistula while minimizing damage to the sphincter muscles to prevent incontinence. The type of surgery depends on the fistula's location, complexity, and its relationship to the sphincter muscles. Fistulotomy: This is the most common surgery for simpler fistulas that don't involve a significant portion of the sphincter muscles. The surgeon cuts along the entire length of the fistula to open it up, turning it into a groove that heals from the inside out as a flat scar. Advancement flap procedure: This is considered when a fistulotomy carries a high risk of incontinence (if the fistula goes through the sphincter muscles).
